A very similar plan to the Blue Preferred plan with the following exceptions: Office visit co-pays are uniform from deductible to deductible and there is a separate $250.00 deductible for Tier 1,2 and 3 prescription drugs. There are seven deductible choices with 80%/20% co-insurance after the deductible.

|

A lower premium PPO plan with Office Visit co-pays limited to Primary Care only with an annual maximum of $160/person. There is also a $500 separate deductible for Tiers 2, 3 and 4 prescription drugs. There are eight deductible choices with 70%/30% Co-insurance after the deductible has been reached.
